To have a Disability or FMLA form completed, please submit it to us using one of the options below.
How to submit your form

- By email — please call our office at (972) 475-8914 and we’ll provide the correct email address for your provider
Please have the form from your employer or disability carrier ready to send with your request. Our provider completes the medical portion; the form itself is not provided by our office.
Fees
There is a $40 fee per form.
Turnaround time
Please allow up to 2 weeks for your form to be completed. If your form needs to be expedited, please call our office at (972) 475-8914 and we’ll connect you with your provider’s medical assistant directly.
