For a copy of your medical records:

The way to request your records depends on the date of your visit.

Visits before April 27, 2026

1. Click the button below to submit your medical records request. Please do NOT submit your request in person.
2. There is no charge for your medical records.
3. You should receive your records within 5 business days of completing your request.
4. If you have any questions, please email medicalrecords@medrecsmanagement.com or call (972) 694-4158.

Visits on or after April 27, 2026

Records from these visits are maintained through Texas Health Physicians Group.

1. To view or download your own records at any time, log in to MyChart at mychart.texashealth.org.
2. If you need a formal or complete records release, or you need help accessing your records, please call our office at (972) 475-8914 and we’ll direct your request.

THIRD PARTY RECORD REQUESTS:

For 3rd parties, such as attorneys and insurance companies, needing to request a copy of a patient’s medical record, please fax your request to (855) 364-4959. If you have any questions, please call us at (972) 694-4158.
